Salt form: Metoclopramide hydrochloride (monohydrate). The hydrochloride salt is the only marketed salt form; all dosing in this monograph refers to metoclopramide hydrochloride, with doses expressed as the base equivalent unless otherwise stated. Salt form distinction is not clinically relevant for prescribing — all Indian formulations contain the hydrochloride salt.

This distinction is fundamental to choosing between these two agents. When EPS risk is a primary concern (elderly, Parkinson’s disease, prior EPS history, children), domperidone is preferred. When a stronger prokinetic effect is needed or parenteral route is required, metoclopramide may be preferred — with appropriate monitoring and duration limitation.

Therapeutic Class: Antiemetic

ℹ️ Metoclopramide has dual pharmacological actions — antiemetic (central D₂ receptor blockade at the chemoreceptor trigger zone [CTZ] and vomiting centre) and prokinetic (peripheral D₂ antagonism + 5-HT₄ agonism in the GI tract). It is classified under “Antiemetic” as the primary therapeutic class because this is its most common prescribing indication in India and its classification in NLEM India 2022 (Section 17.1 — Antiemetics). The prokinetic action is captured in the Subclass and in the Indications section.

Subclass: Substituted Benzamide — Dopamine D₂ Antagonist / 5-HT₄ Agonist (Centrally-Acting)

This subclass distinguishes metoclopramide from:

Domperidone — also a D₂ antagonist but peripherally-acting (does not significantly cross BBB); no 5-HT₄ agonism
Ondansetron / granisetron — 5-HT₃ antagonists (different mechanism entirely)
Prochlorperazine / chlorpromazine — phenothiazine D₂ antagonists (broader receptor profile, more sedation)
Itopride — D₂ antagonist + acetylcholinesterase inhibitor (different dual mechanism)

The “centrally-acting” designation is clinically critical — it explains both the antiemetic efficacy (CTZ blockade) and the EPS liability (nigrostriatal D₂ blockade).

ℹ️ Note: Despite Schedule H status, metoclopramide tablets and syrup are sometimes dispensed without prescription in Indian retail pharmacies, particularly in smaller towns. This is a regulatory compliance issue, not a legal reclassification. The drug remains a prescription-only medicine.

ℹ️ The oral route is the most commonly used in outpatient Indian practice. The IV route is the standard in emergency departments, preoperative settings, oncology units (CINV), and ICUs. The IM route is used when IV access is not available and the patient cannot take oral medication (e.g., actively vomiting patient in a clinic without IV setup).

Biosimilar Status:

Not a biologic — biosimilar classification not applicable. Metoclopramide hydrochloride is a small-molecule chemical drug (substituted benzamide derivative) manufactured by chemical synthesis.

Fixed-dose combinations (FDCs):

ℹ️ FDC landscape for metoclopramide in India is limited. The drug is predominantly prescribed as a single-ingredient product. Some FDCs have been marketed:

⚠️ Brand name confusion alert: In Indian practice, the brand name “Perinorm” (Ipca Laboratories) is the most widely recognised brand for metoclopramide. However, “Perinorm-O” is ondansetron (a completely different drug). This naming similarity has led to documented prescribing and dispensing errors. Always verify the generic name on the dispensed product.

PHARMACOKINETICS

Primary pharmacokinetic parameters:

CYP2D6 polymorphism is clinically significant: CYP2D6 poor metabolisers (~5–10% of Indian population) have reduced clearance and higher steady-state plasma levels — increased risk of adverse effects (especially EPS). CYP2D6 ultra-rapid metabolisers (~1–2% of Indian population) have increased clearance and may have reduced efficacy. Metoclopramide is NOT a clinically significant CYP450 inhibitor or inducer at therapeutic doses. Drug transporter relevance: Metoclopramide is a substrate and weak inhibitor of OCT1 (organic cation transporter 1) and OCT2 (organic cation transporter 2). OCT2 inhibition is relevant for renal tubular secretion interactions (e.g., with metformin, which is also an OCT substrate). No clinically significant P-glycoprotein, OATP, BCRP, OAT, or MATE transporter interactions documented.
Active metabolites	No clinically significant active metabolites. The primary metabolites (N-dealkylated and hydroxylated products) have minimal pharmacological activity.
Half-life (t½)	5–6 hours in adults with normal renal and hepatic function. ⚠️ Prolonged in renal impairment: up to 14–20 hours in severe CKD (eGFR <15 mL/min). ⚠️ Prolonged in neonates: up to 12–24 hours (immature hepatic and renal function). Duration of action	1–2 hours per dose (relatively short). This necessitates multiple daily doses for sustained effect.

Key pharmacological mechanism notes (relevant to PK-PD understanding):

Metoclopramide exerts its clinical effects through three receptor interactions:

Dopamine D₂ receptor antagonism:
- Centrally (CTZ): Blocks dopamine-mediated emetic signals at the chemoreceptor trigger zone (area postrema — outside the BBB) and at the nucleus tractus solitarius → antiemetic effect.
- Centrally (nigrostriatal pathway): ⚠️ Blocks D₂ receptors in the basal ganglia → extrapyramidal side effects (acute dystonia, akathisia, parkinsonism, and with chronic use, tardive dyskinesia).
- Centrally (tuberoinfundibular pathway): Blocks D₂ receptors on lactotrophs → hyperprolactinaemia (galactorrhoea, gynaecomastia, menstrual irregularity).
- Peripherally (GI tract): Blocks inhibitory D₂ receptors on GI smooth muscle and myenteric plexus → releases acetylcholine → enhanced gastric and small intestinal motility (prokinetic effect). Also increases lower oesophageal sphincter (LOS) tone.
5-HT₄ receptor agonism (peripheral):
- Stimulates 5-HT₄ receptors on enteric neurons → further release of acetylcholine → enhanced gastric motility and accelerated gastric emptying. This is the second prokinetic mechanism and distinguishes metoclopramide from domperidone (which lacks 5-HT₄ agonism).
5-HT₃ receptor antagonism (at HIGH doses):
- At high doses (1–2 mg/kg IV — used historically for CINV), metoclopramide weakly blocks 5-HT₃ receptors, contributing to antiemetic action against highly emetogenic chemotherapy. This mechanism is now largely superseded by specific 5-HT₃ antagonists (ondansetron, granisetron).

Non-linear pharmacokinetics: Not clinically significant at standard therapeutic doses. At very high doses (>2 mg/kg, historically used for CINV), the relationship between dose and plasma concentration becomes less predictable, but such doses are rarely used in current practice.

Population Pharmacokinetic Notes:

Steady-state plasma concentrations are approximately 40–50% higher in elderly patients compared to younger adults receiving the same dose. This pharmacokinetic change, combined with the pharmacodynamic increase in dopamine receptor sensitivity with ageing, results in a significantly elevated risk of EPS (particularly tardive dyskinesia) in elderly patients. Dose reduction is mandatory.
Paediatric	Neonates (<28 days): Half-life is markedly prolonged (12–24 hours in preterm; 8–12 hours in term neonates) due to immature hepatic and renal function. Infants and children (1 month – 18 years): Clearance per kg is generally similar to or slightly higher than adults, but children are more susceptible to EPS (particularly acute dystonic reactions), especially adolescent females.
Pregnancy	Increased Vd (expanded maternal plasma volume) and increased renal clearance during pregnancy may result in somewhat lower plasma concentrations. However, the drug crosses the placenta freely. No formal pregnancy PK studies with dose adjustment recommendations exist.
Obesity	The large Vd (3.5 L/kg) distributes into adipose tissue. In obese patients, dosing based on actual body weight may lead to higher total body drug load. For parenteral dosing, consider using ideal body weight (IBW) or a dose cap at the standard adult maximum (10 mg per dose, 30 mg/day) rather than weight-based dosing in obese adults.
Renal impairment	⚠️ Clearance is reduced proportionally to renal function decline. In severe CKD (eGFR <15 mL/min), half-life increases to 14–20 hours. Dose reduction by 50–75% is recommended in severe renal impairment. See RENAL ADJUSTMENT section.
Hepatic impairment	Reduced first-pass metabolism → increased oral bioavailability. Reduced hepatic clearance → prolonged half-life. In cirrhosis, steady-state levels may be significantly elevated. Dose reduction is recommended in moderate-to-severe hepatic impairment. See HEPATIC ADJUSTMENT section.
CYP2D6 poor metabolisers	⚠️ Approximately 5–10% of the Indian population are CYP2D6 poor metabolisers. These individuals have reduced hepatic clearance of metoclopramide, resulting in higher plasma levels and an increased risk of EPS and other dose-dependent adverse effects. Routine CYP2D6 genotyping is not performed before prescribing metoclopramide in Indian practice, but clinicians should be aware that unexpectedly severe EPS at standard doses may indicate poor metaboliser status.
Critical illness / ICU	Altered Vd (fluid overload, hypoalbuminaemia — though the latter is less relevant given the low protein binding of ~30%). Renal function is frequently altered in ICU patients (both AKI and augmented renal clearance). Dose adjustments should be guided by renal function. Gastroparesis and ileus are common in ICU patients — metoclopramide is frequently used as a prokinetic in this setting.

ADULT INDICATIONS + DOSING — FOR CLINICIAN USE ONLY

⚠️ OVERARCHING SAFETY PRINCIPLE — DURATION RESTRICTION:

Regulatory agencies worldwide (EMA 2013, MHRA, TGA) and CDSCO safety communications have recommended that metoclopramide use be restricted to a maximum of 5 days for most indications, and that the maximum daily dose should not exceed 30 mg (or 0.5 mg/kg/day, whichever is lower) in adults. This restriction was imposed due to the cumulative risk of extrapyramidal side effects (EPS) and tardive dyskinesia with prolonged use.

Exceptions to the 5-day rule (where longer use may be justified under specialist supervision with appropriate risk-benefit assessment):

Diabetic gastroparesis (specialist supervision; intermittent courses preferred; continuous use beyond 12 weeks is strongly discouraged)
ICU gastroparesis / feeding intolerance (duration determined by clinical need; stop as soon as enteral feeding is established)
Palliative care (symptom burden may justify prolonged use where alternatives are inadequate)

This duration restriction applies across ALL indications below unless explicitly stated otherwise.

Primary Indications (Approved / Standard in India)

1. NAUSEA AND VOMITING — ACUTE (Non-Specific / Various Causes)

Clinical context: Metoclopramide is one of the most commonly prescribed antiemetics in Indian emergency departments and outpatient practice for acute nausea and vomiting due to gastroenteritis, medication side effects, post-procedural emesis, uraemia, and non-specific causes. It acts as an antiemetic through central D₂ receptor blockade at the CTZ and additionally promotes gastric emptying, making it useful when nausea is accompanied by gastric stasis.

Mandatory Clinical Notes:

When to prefer this drug over alternatives:
- Prefer metoclopramide when nausea/vomiting is accompanied by gastric stasis or gastroparesis — the dual antiemetic + prokinetic action is specifically advantageous (e.g., diabetic patient with nausea, post-surgical gastroparesis, opioid-induced nausea with gastroparesis).
- Prefer metoclopramide over ondansetron when prokinetic effect is desired in addition to antiemesis. Ondansetron has no prokinetic activity and may worsen constipation (relevant in opioid-treated patients and ICU patients).
- Prefer over prochlorperazine when a less sedating antiemetic with prokinetic action is desired. Prochlorperazine is more sedating and has no significant prokinetic effect.
- Domperidone comparison: Domperidone has a lower risk of EPS (does not cross BBB significantly) and is therefore preferred in elderly patients, patients with Parkinson’s disease, children (where EPS risk is high), and patients with a history of EPS or dystonic reactions. However, domperidone is available only as an oral formulation in India (injection withdrawn due to QT prolongation risk) — so when a parenteral antiemetic with prokinetic action is needed, metoclopramide is the only option in this class.
When NOT to use even though technically indicated:
- ⛔ Patients with Parkinson’s disease or those receiving dopaminergic therapy (levodopa, pramipexole, ropinirole) — metoclopramide directly antagonises dopaminergic transmission and will worsen parkinsonian symptoms. Use domperidone instead (does not cross BBB).
- ⛔ Patients with epilepsy — metoclopramide may lower the seizure threshold.
- ⛔ Patients with phaeochromocytoma — risk of hypertensive crisis (metoclopramide can release catecholamines from the tumour).
- ⚠️ Patients with prior EPS or dystonic reaction to any dopamine antagonist — high risk of recurrence.
- ⚠️ Young women (15–35 years) — the demographic group at highest risk of acute dystonic reactions to metoclopramide. Consider ondansetron or domperidone as first-line alternatives.
- ⚠️ Elderly patients — prefer domperidone (lower EPS risk) or ondansetron (no EPS risk). If metoclopramide must be used, reduce dose to 5 mg TDS.
- ⚠️ Suspected bowel obstruction or perforation — prokinetic effect can worsen mechanical obstruction. Rule out surgical abdomen before prescribing.
NLEM India status: ✅ Listed in NLEM India 2022 — Section 17.1 (Antiemetics). Both tablets (10 mg) and injection (5 mg/mL) are included.
Typical time to expected clinical response:
- IV: 1–3 minutes (very rapid antiemetic onset).
- IM: 10–15 minutes.
- Oral: 30–60 minutes.
- If no response within 30 minutes (IV) or 60 minutes (oral), consider alternative antiemetic (ondansetron 4 mg IV, or prochlorperazine).
Criteria for treatment failure and switching:
- If vomiting persists despite 3 doses of metoclopramide over 24 hours, reassess:
  - Is there a surgical cause? (obstruction, appendicitis, pancreatitis, cholecystitis)
  - Is the emetic stimulus resistant to D₂ blockade? (chemotherapy, motion sickness, raised ICP — different antiemetic mechanisms needed)
- Switch to ondansetron (4–8 mg IV/oral TDS) for refractory vomiting.
- For vestibular/motion sickness-related vomiting, metoclopramide is not effective — use promethazine, cinnarizine, or hyoscine hydrobromide.
- For raised ICP-related vomiting, metoclopramide is not first-line — manage the underlying cause; consider dexamethasone.
Mandatory baseline investigations:
- No mandatory laboratory investigations for short-term (≤5 days) antiemetic use.
- MANDATORY clinical assessment: Rule out surgical abdomen (peritonitis, obstruction). Rule out bowel perforation. Enquire about Parkinson’s disease, epilepsy, prior EPS.
- RECOMMENDED: Serum electrolytes (hypokalaemia and hypomagnesaemia lower seizure threshold and increase QT risk); renal function (dose reduction needed in CKD).
Specialist initiation: Not required for short-term acute antiemetic use. Any registered medical practitioner can prescribe. PHC/CHC-level prescribing is appropriate for ≤5 days’ use.
Indian guideline source: NLEM India 2022 lists metoclopramide as an essential antiemetic. API Textbook of Medicine (current edition) references metoclopramide for nausea/vomiting management. No specific ICMR guideline addresses antiemetic selection.
Key disease-specific safety warning: ⚠️ Acute dystonic reactions can occur after even a SINGLE dose, particularly in young women and children. Classic presentation: oculogyric crisis (eyes rolling upward), torticollis (neck twisted to one side), trismus (jaw clenching), opisthotonos, tongue protrusion. Onset typically within 24–72 hours of starting the drug. Treatment: IV/IM promethazine 25 mg or IV/IM diphenhydramine 50 mg (benztropine 1–2 mg IV is an alternative but less available in India). Response is usually dramatic within 5–10 minutes.
Common dose adjustment scenarios:
- Elderly (≥60 years): Reduce dose to 5 mg per dose, up to TDS. Max 15 mg/day. Monitor for EPS.
- Renal impairment (eGFR <30): Reduce dose by 50%. See RENAL ADJUSTMENT.
- Hepatic impairment (Child-Pugh B/C): Reduce dose by 50%. See HEPATIC ADJUSTMENT.
- Low body weight (<60 kg): Consider 5 mg per dose rather than 10 mg.
- Concurrent opioids: Monitor for additive sedation. Metoclopramide can help counteract opioid-induced gastroparesis — a practical advantage.

2. GASTROPARESIS — DIABETIC AND NON-DIABETIC (Prokinetic Use)

Clinical context: Metoclopramide is the only prokinetic drug with robust clinical trial evidence and regulatory approval for diabetic gastroparesis. It accelerates gastric emptying through dual D₂ antagonism (releasing cholinergic activity in the myenteric plexus) and 5-HT₄ agonism (directly stimulating enteric neurons to release acetylcholine). This makes it pharmacologically more potent as a prokinetic than domperidone (which has D₂ antagonism only) or mosapride (which has 5-HT₄ agonism only).

⚠️ Duration of use for gastroparesis:

Initial course: 2–4 weeks is recommended to assess response.

Domperidone comparison in ICU: Domperidone is impractical in ICU patients because it is oral-only — many ICU patients cannot swallow. Metoclopramide’s IV availability is the key advantage in the ICU setting. If the patient can take oral medication via NG tube, domperidone 10 mg TDS via NG may be considered as an alternative (lower EPS risk) — crush tablets and suspend in water for NG administration.
⚠️ EPS in ICU patients may be masked by sedation, confused with delirium, or attributed to other neurological causes. Maintain a high index of suspicion for EPS in any ICU patient receiving metoclopramide who develops new involuntary movements or worsening agitation.

PAEDIATRIC DOSING (Specialist Only)

⛔ All paediatric use of metoclopramide must be under specialist supervision (paediatrician, paediatric gastroenterologist, paediatric oncologist, or paediatric intensivist). This drug carries a HIGHER risk of extrapyramidal side effects in children than in adults.

General Notes:

⚠️ EPS risk in children: Children and adolescents are significantly more susceptible to acute dystonic reactions from metoclopramide than adults. The risk is highest in:
- Children <1 year (immature BBB, immature dopaminergic pathways)
- Adolescent females (10–19 years)
- Dehydrated or febrile children (increased BBB permeability)
- Children receiving concurrent medications that lower the seizure threshold or have dopaminergic activity
- CYP2D6 poor metabolisers (may not be known in advance)
⚠️ Age restriction: Per European Medicines Agency (EMA 2013) recommendations (reflected in many CDSCO product inserts for metoclopramide):

Safety monitoring requirements specific to paediatric use:
- ⚠️ Monitor closely for EPS after EVERY dose — especially during the first 24–72 hours of treatment. Educate caregivers to recognise: neck stiffness/twisting (torticollis), eye rolling upward (oculogyric crisis), jaw clenching (trismus), tongue protrusion, generalised muscle stiffness, restlessness/agitation (akathisia).
- Monitor heart rate (metoclopramide can cause transient supraventricular tachycardia in neonates and infants).
- Monitor for excessive sedation (more pronounced in young children).
- Monitor for methaemoglobinaemia in neonates (particularly preterm infants) — metoclopramide-induced methaemoglobinaemia is a rare but documented serious adverse effect in neonates due to immature methaemoglobin reductase activity.
Minimum age: ≥1 year for standard paediatric use. Neonatal use (<28 days) is exceptional and restricted to NICU settings (see Neonatal Dosing below).
Minimum weight: No strict minimum weight threshold, but weight-based dosing must be used to avoid overdose. The concentration of oral drops (4 mg/mL in some brands) versus syrup (1 mg/mL) is a critical source of dosing errors — verify the formulation concentration before calculating volume.
Formulation suitability for children:
- Oral syrup/solution (5 mg/5 mL = 1 mg/mL): Suitable for paediatric dosing. Measure with a graduated oral syringe — NOT a household teaspoon (which delivers inconsistent volumes). Available in India from multiple manufacturers. Palatability is generally acceptable (flavoured).
- Oral drops (4 mg/mL — some brands): ⚠️ HIGH CONCENTRATION — extreme care needed. A 4-fold dosing error can occur if drops are confused with syrup (1 mg/mL). Always verify the concentration on the product label before calculating the dose. Oral drops are suitable for infants and young children where small volumes are needed.
- Tablets (10 mg, 5 mg): Suitable for children ≥6 years who can swallow tablets whole. The 5 mg tablet is useful for dose adjustment. Tablets should NOT be crushed (no stability data for crushed form; bitter taste).

- Neonates (especially preterm): Half-life markedly prolonged (12–24 hours in preterm; 8–12 hours in term neonates) due to immature hepatic conjugation/oxidation and reduced renal clearance. Steady-state takes longer to achieve, and drug accumulation is a significant risk with standard dosing intervals.
- Infants (1–12 months): Half-life approximately 4–6 hours (approaching adult values by 6 months of age in term infants). Clearance per kg is variable.

Dosing method: Weight-based (mg/kg) — mandatory for all paediatric dosing. Fixed-dose tablets should NOT be used without verifying that the dose falls within the mg/kg range.

Adolescent transition: Children ≥15 years AND ≥60 kg may use adult dosing (10 mg per dose, max 30 mg/day). For adolescents 12–18 years weighing <60 kg, continue weight-based dosing with adult maximum ceiling (10 mg per dose, 30 mg/day).

Neonatal Dosing (<28 days of life)

⛔ Neonatal use — NICU supervision only

Metoclopramide use in neonates is NOT standard practice and is associated with significant risks. It is used in some Indian NICUs for specific indications under close specialist supervision.

⚠️ Special neonatal risks:

Methaemoglobinaemia: Neonates (especially preterm <34 weeks gestation) have reduced levels of NADH-methaemoglobin reductase and higher levels of fetal haemoglobin, making them susceptible to drug-induced methaemoglobinaemia. Metoclopramide is a documented cause. Monitor SpO₂ and consider co-oximetry if clinical suspicion arises (cyanosis unresponsive to supplemental oxygen).
Prolonged half-life: 12–24 hours in preterm, 8–12 hours in term neonates. Risk of accumulation with standard dosing intervals.
EPS: Risk exists even at low doses. Immature BBB in neonates allows greater drug penetration.
Cardiac effects: Reports of supraventricular tachycardia and QT prolongation in neonates.

Mandatory Clinical Notes (Paediatric-specific):

When to prefer over alternatives:
- Ondansetron is preferred as the first-line paediatric antiemetic for acute gastroenteritis-related vomiting (Cochrane review evidence, IAP recommendation). Metoclopramide is second-line.
- Prefer metoclopramide when prokinetic effect is additionally needed (e.g., vomiting with gastric stasis, diabetic adolescent with gastroparesis, postoperative gastroparesis).
- Prefer metoclopramide when parenteral antiemetic with prokinetic action is needed (domperidone is oral-only in India).
- Domperidone comparison in paediatrics: Domperidone (0.25 mg/kg per dose, TDS) is generally preferred over metoclopramide in children because of the significantly lower EPS risk. - ⚠️ Dehydrated or febrile children — increased BBB permeability raises EPS risk. Correct dehydration before considering metoclopramide.

Safety warning: ⚠️ Acute dystonic reactions in children are a medical emergency. MISSED DOSE / DELAYED DOSE GUIDANCE

Context-specific guidance by dosing frequency and clinical use:

1. PRN / Acute Use (single-dose or short-course antiemetic for acute nausea/vomiting):

Not applicable — metoclopramide for acute nausea/vomiting is typically given as needed (PRN). There is no scheduled dosing to “miss.”

2. Three-times-daily (TDS) Scheduled Dosing (e.g., for gastroparesis, GORD, or regular antiemetic use):

ℹ️ Actually, drug holidays are encouraged for this drug. Intermittent use with periodic breaks reduces cumulative exposure and lowers tardive dyskinesia risk. A drug holiday of ≥2 weeks between courses is a reasonable approach for chronic indications (gastroparesis).

5. Palliative Care / Continuous Use:

In palliative care settings where metoclopramide is used continuously for chronic nausea, missed doses may lead to return of symptoms. Resume at the previous dose when next due. No re-titration needed.

RENAL ADJUSTMENT

eGFR formula specification: Dose adjustment recommendations for metoclopramide are based on Cockcroft-Gault creatinine clearance (CrCl) from the original pharmacokinetic studies. CKD-EPI eGFR may be used for practical clinical decision-making, but note that CKD-EPI eGFR values may overestimate renal function compared to Cockcroft-Gault CrCl, especially in elderly patients — potentially leading to under-adjustment.

Key pharmacokinetic consideration: Approximately 85% of a metoclopramide dose is excreted renally (20–25% as unchanged drug, remainder as metabolites). Renal impairment significantly reduces clearance and prolongs the half-life — from ~5 hours in normal renal function to ~14–20 hours in severe CKD (eGFR <15). This increases steady-state plasma concentrations and elevates the risk of dose-dependent adverse effects, particularly EPS and sedation.

HEPATIC ADJUSTMENT

Primary clearance pathway: Hepatic metabolism accounts for the majority of metoclopramide clearance — via N-dealkylation (partially CYP2D6-dependent), oxidative hydroxylation, and conjugation (glucuronidation, sulfation). The drug has a moderate hepatic extraction ratio. In hepatic impairment: (a) reduced first-pass metabolism increases oral bioavailability (already variable at 30–100%); (b) reduced hepatic clearance prolongs half-life; © the net effect is significantly elevated steady-state plasma concentrations.

No formal Child-Pugh-based dosing data exists for metoclopramide. The following guidance is based on pharmacokinetic principles and specialist practice:

Concurrent hepatotoxin note: Metoclopramide itself is NOT hepatotoxic — it does not cause drug-induced liver injury. No specific concern regarding additive hepatotoxicity when co-administered with hepatotoxic drugs (rifampicin, isoniazid, pyrazinamide, methotrexate, valproate, antiretrovirals).

However, note the following clinically important interactions in the hepatic impairment context:

⚠️ Metoclopramide + lactulose in hepatic encephalopathy: Metoclopramide accelerates GI transit, which may reduce the contact time of lactulose with colonic bacteria, potentially reducing lactulose’s efficacy in hepatic encephalopathy. This is a theoretical concern with limited clinical evidence, but awareness is warranted. Monitor encephalopathy grade if metoclopramide and lactulose are co-prescribed. If prokinetic is needed in a patient with hepatic encephalopathy, consider whether the prokinetic benefit outweighs the potential lactulose efficacy reduction.
⚠️ EPS mimicking hepatic encephalopathy: Metoclopramide-induced movement disorders (dystonia, akathisia, parkinsonism) can be mistaken for worsening hepatic encephalopathy in cirrhotic patients, leading to unnecessary escalation of encephalopathy treatment instead of stopping the offending drug. Maintain a high index of suspicion — if new movement abnormalities or worsening confusion develop after starting metoclopramide in a cirrhotic patient, consider drug-induced EPS as the cause.

CONTRAINDICATIONS

⛔ Absolute contraindications — the drug must NEVER be used in these situations:

This is a well-documented and potentially fatal interaction. The mechanism involves blockade of inhibitory D₂ receptors on chromaffin cells, releasing catecholamine secretion from inhibitory control. ⛔ Also contraindicated in patients with paraganglioma.
⛔ Mechanical gastrointestinal obstruction, perforation, or haemorrhage	Metoclopramide’s prokinetic action increases GI motility and intraluminal pressure. In the presence of mechanical obstruction, this can cause or worsen perforation. In the presence of active GI haemorrhage, increased motility may worsen bleeding. In perforation, the drug is ineffective and delays surgical management by masking symptoms.
⛔ Epilepsy or seizure disorder	Metoclopramide lowers the seizure threshold by modulating central dopaminergic and GABAergic transmission. In patients with known epilepsy, even at therapeutic doses, the drug can precipitate seizures. This contraindication is present in CDSCO product inserts. ℹ️ Note: Some references list epilepsy as a “caution” rather than absolute contraindication — however, the CDSCO product insert for most Indian brands lists it as a contraindication. This monograph follows CDSCO labelling. If the clinical situation is exceptional (e.g., CINV in a patient with well-controlled epilepsy on therapeutic antiepileptics), use ONLY under specialist supervision with documented risk-benefit assessment.
⛔ Prolactin-dependent tumour (prolactinoma)	Metoclopramide blocks D₂ receptors on pituitary lactotrophs → increases prolactin secretion → can stimulate growth of prolactin-dependent tumours. Absolutely contraindicated in patients with known prolactinoma or prolactin-secreting pituitary adenoma.
⛔ Concurrent use with levodopa or dopamine agonists	See MAJOR DRUG INTERACTIONS. Metoclopramide directly antagonises dopaminergic transmission, negating the therapeutic effect of levodopa, pramipexole, ropinirole, rotigotine, cabergoline, and bromocriptine. This combination is pharmacodynamically contraindicated — both drugs cancel each other’s primary effect. In Parkinson’s disease patients, metoclopramide will worsen motor symptoms.
⛔ History of tardive dyskinesia	Patients who have previously developed tardive dyskinesia (TD) from any dopamine-blocking drug (metoclopramide, antipsychotics, prochlorperazine) are at very high risk of recurrence — even with short courses. TD recurrence can be rapid and may be irreversible. Use antiemetics without D₂ blocking activity (ondansetron, dexamethasone).
⛔ History of neuroleptic malignant syndrome (NMS)	Patients with a history of NMS from any dopamine-blocking agent are at increased risk of recurrence. Metoclopramide has been documented to cause NMS (see Serious Adverse Effects).
⛔ Neonates <1 year (relative — see text)	Per EMA 2013 recommendations reflected in updated CDSCO product inserts: metoclopramide is not recommended in children below 1 year of age. Risk of EPS, methaemoglobinaemia (neonates), and prolonged drug effects (immature metabolism) outweigh benefits in almost all scenarios. Hypertension	Metoclopramide may cause transient blood pressure changes (usually mild). In patients with phaeochromocytoma, it is absolutely contraindicated (see above). In patients with essential hypertension, standard use is safe with routine BP monitoring.
G6PD deficiency	Data limited. No well-documented haemolytic risk, but metoclopramide-induced methaemoglobinaemia (rare) may be theoretically more significant in G6PD-deficient patients. Relevant primarily in neonates and infants.

PREGNANCY

The general highest-risk period for organogenesis-related teratogenicity is weeks 3–8 post-conception (weeks 5–10 gestational age) — metoclopramide does not appear to increase risk during this window.

Preferred alternatives in Indian obstetric practice	First-line for NVP: Doxylamine 10 mg + Pyridoxine 10 mg (available as Doxinate in India); widely used, well-studied, low risk. Second-line: Ondansetron 4 mg oral or IV — commonly used in Indian obstetric practice; large human safety data available (though some guidelines advise caution in first trimester due to isolated cardiac malformation concerns — not confirmed in largest studies). Fertility Effects:

⚠️ Prolactin-related effects on fertility: Metoclopramide raises prolactin levels by blocking pituitary D₂ receptors. Hyperprolactinaemia can cause:

In women: Amenorrhoea, oligomenorrhoea, anovulation, galactorrhoea → may impair fertility. If a woman of reproductive age is using metoclopramide regularly and develops menstrual irregularity, check serum prolactin. Discontinuation of metoclopramide usually normalises prolactin and restores ovulation.
In men: Gynaecomastia, decreased libido, erectile dysfunction, impaired spermatogenesis (with prolonged hyperprolactinaemia) → may impair fertility.

These effects are reversible upon discontinuation in most cases. No specific washout period before planned conception is required — discontinue the drug when no longer clinically needed, and fertility should normalise within 1–2 menstrual cycles (women) or 2–3 months (men).

ℹ️ Paradoxical use as a galactogogue: The prolactin-raising effect is exploited off-label to stimulate lactation. See Secondary Indication 3 in Part 2.

LACTATION

Preferred alternatives during lactation	Domperidone (10 mg TDS for 7–14 days) is the preferred drug for lactating mothers who need either an antiemetic/prokinetic OR a galactogogue because: (a) lower transfer into breast milk (RID ~0.01–0.04% — significantly lower than metoclopramide); (b) does not cross the infant’s BBB even if ingested via milk (quaternary-like peripheral selectivity); © equivalent prolactin-raising effect for galactogogue purpose; (d) no CNS side effects in mother (no depression, no EPS). Ondansetron is the preferred antiemetic during lactation if the prokinetic effect is not needed (negligible milk transfer, no CNS or prolactin effects).
Timing advice	If metoclopramide must be used during breastfeeding: take the dose immediately after completing a breastfeed. This ensures that the next feed occurs at the trough of milk drug concentration (approximately 4–6 hours later), minimising infant exposure.

Effect on milk production:

⚠️ Metoclopramide INCREASES milk production — this is a well-documented pharmacological effect mediated by prolactin elevation. This effect is the basis for its off-label use as a galactogogue (see Secondary Indication 3 in Part 2). The increase in milk production is typically evident within 2–5 days of starting the drug and is sustained during continued use. Milk production returns to baseline within a few days of discontinuation.

ℹ️ This is an intended effect when the drug is prescribed as a galactogogue, but it is also important to be aware of when the drug is prescribed for other indications (e.g., nausea) in a lactating mother — breast engorgement may occur if the mother is not prepared.

Temporary incompatibility guidance:

Not applicable — the drug is considered compatible with breastfeeding for short courses. No need to withhold breastfeeding, pump and discard, or time doses around feeds (though timing after feeds is preferred to minimise infant exposure). For a single dose (e.g., preoperative aspiration prophylaxis), breastfeeding can continue without interruption.

ELDERLY

Definition: ≥60 years (consistent with Indian Census and National Programme for Health Care of the Elderly).

⚠️ Metoclopramide carries the HIGHEST RISK of serious adverse effects in the elderly population compared to younger adults. The combination of pharmacokinetic changes (reduced clearance), pharmacodynamic changes (increased D₂ receptor sensitivity), and the high prevalence of comorbidities (CKD, polypharmacy, cognitive impairment, Parkinson’s disease) in elderly Indian patients makes metoclopramide a high-risk drug in this population.

Key risks in elderly	1. Tardive dyskinesia (TD) — the most feared adverse effect. Risk is 5–10 times higher in elderly patients compared to younger adults. TD incidence in elderly receiving chronic metoclopramide is estimated at 15–30% (compared to ~1–5% in younger adults). TD may be irreversible in up to 50% of elderly patients. Elderly women are at the highest risk. 2. Parkinsonism — drug-induced parkinsonism from metoclopramide can be indistinguishable from idiopathic Parkinson’s disease. Misdiagnosis is common — the elderly patient may be started on levodopa for “Parkinson’s disease” when the actual cause is metoclopramide. Always take a thorough drug history before diagnosing new-onset parkinsonism in elderly patients. 3. Akathisia — motor restlessness that may be misinterpreted as agitation, anxiety, or behavioural disturbance. May lead to inappropriate prescribing of benzodiazepines or antipsychotics, compounding the problem. 4. Falls — from drug-induced parkinsonism (bradykinesia, rigidity, postural instability), akathisia (restless pacing → falls), sedation (drowsiness → falls), and orthostatic hypotension. Falls in elderly → hip fractures → significant morbidity and mortality. 5. Sedation and cognitive impairment — drowsiness, confusion, impaired concentration. May worsen pre-existing cognitive impairment or be mistaken for dementia progression. 6. Depression — elderly patients are already at higher risk of depression; metoclopramide can precipitate or worsen depressive episodes. 7. Prolactin-related effects — gynaecomastia (embarrassing and distressing for elderly men), galactorrhoea.

Anticholinergic Cognitive Burden (ACB):

ℹ️ Metoclopramide has NO significant anticholinergic properties. ACB Score: 0 (no anticholinergic activity).

In fact, metoclopramide has the opposite of anticholinergic action on the GI tract — it is a pro-cholinergic prokinetic (increases acetylcholine release from the myenteric plexus via D₂ blockade and 5-HT₄ agonism). This means metoclopramide does NOT contribute to anticholinergic burden and does NOT cause the anticholinergic adverse effects (dry mouth, constipation, urinary retention, cognitive impairment from central cholinergic blockade) that characterise drugs like dicyclomine, hyoscine, amitriptyline, and oxybutynin.

However, metoclopramide’s central D₂ blockade effects (EPS, TD, depression, sedation) are equally or more dangerous than anticholinergic cognitive burden in elderly patients — so the drug remains high-risk despite having zero ACB.

Beers Criteria / STOPP-START Relevance:

Gradual onset	ℹ️ For most immediate-release drugs, the effect is clinically minor. ⚠️ For modified-release/sustained-release formulations: be cautious — metoclopramide-accelerated transit may impair the controlled-release mechanism. Monitor for signs of increased drug effect (toxicity) or decreased effect (subtherapeutic levels). For narrow therapeutic index modified-release drugs (theophylline SR, lithium SR, verapamil SR, carbamazepine SR), monitor drug levels.
CYP2D6 inhibitors (fluoxetine, paroxetine, bupropion, quinidine, duloxetine, terbinafine)	CYP2D6 is involved in metoclopramide metabolism. COMMON ADVERSE EFFECTS

ℹ️ The adverse-effect profile of metoclopramide is dominated by two pharmacological consequences of central D₂ receptor blockade: extrapyramidal symptoms (EPS) and hyperprolactinaemia. Unlike domperidone (which minimally crosses the BBB), metoclopramide freely enters the CNS — making CNS-related adverse effects the primary safety concern.

Frequency classification source: Derived from clinical trial data, post-marketing surveillance, CDSCO product inserts, and published pharmacovigilance analyses. Exact incidences vary by population, dose, duration, and route of administration.

Very Common (≥10%):

When to Stop Monitoring

Short-term use (≤5 days): Monitoring can be discontinued once the drug is stopped and the patient is assessed for any residual adverse effects at the next clinical visit.

“This medicine stops vomiting and also helps your stomach work faster. If food is staying in your stomach too long (making you feel full, bloated, or nauseated), this medicine helps push the food forward into your intestines.”

How to take it:

Tablets/Syrup: “Take this medicine 30 minutes before your meals and, if your doctor has prescribed a bedtime dose, before going to sleep. Swallow the tablet with a glass of water. If you are using the syrup, measure the dose carefully with the measuring cup or syringe provided — do not use a regular kitchen spoon.”
Injection: “This will be given to you by a doctor or nurse. You do not need to do anything.”

What to do if you miss a dose:

“If you forget a dose, take it as soon as you remember — but only if your next dose is more than 3 hours away. If your next dose is within 3 hours, skip the missed dose. Never take two doses at once. If you forget multiple doses, do not try to catch up — just continue with your regular schedule.”

Common side effects to expect:

“You may feel sleepy or tired — this usually improves after the first day or two. If it continues, tell your doctor. Some people feel restless or unable to sit still — if this happens, tell your doctor as the dose may need to be reduced or the medicine may need to be changed. You may notice slightly looser stools — this is because the medicine speeds up your digestion. Drink enough water.”

Breast milk leaking (in women who are not breastfeeding) or breast swelling (in men)

These side effects are treatable, but you must get medical help quickly."

“This medicine is usually prescribed for a short time only (a few days). It is NOT a medicine that you take for months or years. If your doctor has prescribed it for a stomach emptying problem (gastroparesis), they will review regularly whether you still need it. Never continue this medicine on your own beyond the prescribed duration. You can stop it anytime without gradually reducing the dose.”

CLINICAL PEARLS

💡 1. The “5-day rule” — know it, enforce it, document it. Metoclopramide should be used for a maximum of 5 days for most indications. This is the single most important prescribing rule for this drug. Tardive dyskinesia risk increases with cumulative exposure, and TD can be irreversible. Before prescribing, set a stop date. At each refill, ask: “Is this still needed?” For gastroparesis (the only indication where >5 days may be justified), document informed consent regarding TD risk and reassess at 2 weeks, 4 weeks, and 12 weeks. 💡 2. IV metoclopramide must be given SLOWLY — at least 3 minutes, ideally 15 minutes. Rapid IV push is the single biggest modifiable risk factor for acute dystonic reactions with metoclopramide. Slow IV bolus (≥3 minutes) or dilution in 50 mL NS infused over 15 minutes significantly reduces peak plasma concentration spikes and lowers EPS risk. This is especially critical in children, young women, and elderly patients. Many Indian emergency departments give metoclopramide as a rapid IV push — this practice should be corrected. 💡 3. Myth vs Fact — “Metoclopramide is just a simple anti-vomiting medicine.”
Myth: Metoclopramide is a benign, routine antiemetic that can be prescribed freely without significant risk.
Fact: Metoclopramide is a centrally-acting dopamine antagonist with a US FDA Black Box Warning equivalent for tardive dyskinesia risk. It is one of the most common causes of drug-induced movement disorders worldwide. In India, it is often prescribed casually (including at PHC/CHC level) without awareness of the EPS/TD risk, without duration limits, and without monitoring. It is pharmacologically closer to an antipsychotic (haloperidol) in its central D₂ blocking action than it is to ondansetron. Treat it with the respect its pharmacology demands. 💡 4. Drug-induced parkinsonism from metoclopramide is commonly MISDIAGNOSED as Parkinson’s disease in elderly Indian patients. Always take a complete drug history (including metoclopramide, prochlorperazine, and other D₂ antagonists) before diagnosing new-onset Parkinson’s disease in any elderly patient. Metoclopramide-induced parkinsonism is reversible on stopping the drug (usually within 2–4 weeks, sometimes up to 3–6 months) — but only if the diagnosis is made. Starting levodopa for misdiagnosed drug-induced parkinsonism while continuing metoclopramide is a pharmacological catastrophe (mutual antagonism). 💡 5. For elderly patients needing an antiemetic: ondansetron first, domperidone second, metoclopramide LAST. This hierarchy is based on the dramatically higher EPS/TD risk in elderly patients. Ondansetron (4 mg oral or IV) has ZERO EPS risk and is the safest first-line antiemetic in elderly. Domperidone (10 mg TDS oral) has minimal EPS risk but carries QT prolongation concerns (monitor). Metoclopramide should be reserved for elderly patients only when both alternatives have failed AND a prokinetic effect is specifically needed AND the parenteral route is required.
